 I have three documents that will need to be filled out and/or signed when you buy a puppy from me.  Click on the document (below) that you are interested in.  Download it and complete it.  Then mail it to my address below (and at the top of the document) or attach it to an email.  If the link does not work, I can either mail it to you or fax it to you if you have a fax number.

When you pick up your puppy, I will have the purchase contract for you to sign and receive a copy of it.
